Use the Category Lists below to help you quickly find specific documents provided in the website.

IF YOU HAVE A DIAL UP MODEM, SOME OF THESE DOCUMENTS CAN TAKE UP TO FIVE MINUTES TO DOWNLOAD.

To open documents that have "MS Word" after them, you will need to have MS Word or Office software loaded on your computer.

To open documents that have "PDF" after them, you will need an Adobe Acrobat Reader, which can be downloaded for free from the Adobe Website (click here to link to the Adobe site). Simply click on the link and follow the instructions on your screen to download the reader. Once the Reader is downloaded, come back to this page and click on the forms you want. If you want to download the Adobe Acrobat Reader, but aren't feeling brave about the task, call Sheila Kruse at 586-2337 for semi-technical back-up.

Remember, if you are having trouble downloading materials, use the Request Form Page or call Janet Cook at 533-7427 to ask for materials to be mailed to you.
